Should I get lower premolar extractions + massater botox?
Or would this nerf.

Really bad TMJ issues + tooth wear from clenching. Also my cheekbones used to look more prominent before my daytime bruxism became so bad

I would get that but I wouldn't extract premolar

What surgery would fix this?
bimax. if you extract 100 percent you gonna look worse also you kinda have short face syndrome get that fixed while at it

I’ve got shit TMD as well, ended up qualifying for surgery including joint replacement & DJS for free. Surgery probably the last step you want to take to treat TMD tho

I'm already getting functional rhinosepto soon, I'll probably need to wait until 18 for anything cosmetic to reduce projection.
 
I’ve got shit TMD as well, ended up qualifying for surgery including joint replacement & DJS for free. Surgery probably the last step you want to take to treat TMD tho
DJS would probably save me facially. This is my ceph, does it/did it resemble yours at all?

I agree, counting down the days until my operation lol. Your bite/skeletal class looks pretty normal (I’m a class II), so if you do get djs it might be purely aesthetic. For TMD the procedures I found the most helpful were botox & steroid injections.

I think my skeletal class is normal; however, I do have "short face syndrome" (Hypodivergent growth), and my natural tooth resting position makes it extremely easy to clench.

Yeah bruxism puts a real load on the joint & hypertrophies the masseters, TMD speedrun

Yeah, I always hear ringing when I chew, or clench, or open my jaw, or move the jaw side to side. I'm also noticing more headaches recently. + The tooth wear
