A orthodontist I went to said I would be a strong candidate for a MARPE, and the MARPE itself he said would help my maxilla a lot. Would it also help my mandible is arguably a greater problem? Online I've seen mixed reviews and so I don't know. My mandible is recessed, but that's largely because its just being pushed down by my recessed maxilla. Any ideas?

Marpe won't do anything at all for your mandible

Here’s some photos of me. Please give opinions of if marpe can ascend or descend me at 20:

Your midface isn't too bad I don't see how you could profit much from marpe compared to trimax

lol maybe mandible autorotation that's a stretch it still wont do shit
it can move the mandible in a better position, but yeah its not very significant

I’d be happy with any minor changes since the breathing change could also help me.

Do you guys think OP3-4 injected into the tmj or mandibular condyle could promote growth and fix the issue assuming ccw rotation is instituted even slightly from marpe. This way my mandible can catchup with my maxilla

U would hugely benefit from both marpe and jaw surgery

You've got 2 options.
- Jaw surgery
- cope

MARPE isn't going to do anything significant to your mandible. So get double jaw surgery counterclockwise rotation with a posterior downgraft to lengthen and rotate your lower third.

skip the marpe, braces in, decomp and get your segmental lefort, maxilla/mandible advancement/rotation

rotation won't do much for the lower third it only rotates the OP

Marpe will give u an not aligned bite with ur mandible so i suppose ur mandible will also need to get wider (surgery/outside force). So that ur bite will be normal but i it wont hapoen from its own

I know a bimax would save me, but I can’t get one.

My options are marpe (would it make me uncanny since my lower third is a little narrow (gum might fix?))
And/or
Op3-4 tmj injections to mimic epigenetic results from chewing

He said I can 4mm and tip out my lower teeth to align

Like he can just move the teeth to align with a wider maxilla
Okay. Wider maxilla will give u better fwhr but lower bizygomatic/bigonial width ratio

So my zygos would be too wide? Could I compensate with larger masseyers?

Im at the same problem. U can look at my thread which i didnt respond to yet
Thread 'How can I make my orthodontist (Zahnarzt) to give me RPE (if still possible at 17) or MARPE?'

Autorotation will slightly help but probably not enough.

That’s why I’m thinking we need to do op3-4 tmj injections to grow our mandible. It can still grow but not enough from chewing so we need to accelerate it. It doesn’t sound like anybody’s tried this before but surely it should work

Where have you read about op3-4 injections to grow the mandible? I'm curious. There was a study iirc on rabbits where they injected their condyles with HGH.

Well I found out that the way the homeoblock works is the stimulate the jaw into activating the epigenetic pathways that grow the mandible from chewing. This same rationale is held by people who emphasize a diet of hard (primal) foods because it tells the jaw to grow more. It's also studied that ramus length is positively correlated with massetter size/strength.

The idea is that you might be able to mimic the epigenetic pathway of chewing, with peptides. This is where OP3-4 may come in where I a small, yet strong injection into the tmj or mandibular condyle could incite growth and speed it up alongside hard chewing. We could also try the PAP Polypeptide.

There's not much documentating on this and I don't think anybody on the forum has tried it, but the science is there and it's only a matter of time until somebody experiments with this. If it works, this means one could fix their mandibular growth and create the same result as a bsso, just without the long painful recovery periods

If you do it let me know. I'm curious to see your progress.

Apparently abaloparatide is a better solution as op3-4 would only work if the bone was already damaged, but abaloparatide is a osteoperosis medication that can grow actual bone

Do you wanna grow bone though? I thought the condyles were mostly cartilage hence the rabbit study using HGH because it promotes cartilage growth. And by growing the condyles that should push your lower jaw forwards no?

Yeah well maybe both then because I believe the cartilage gets replaced by bone during the growth process

Uff we are going out of my knowledge. I dont know what op3-4 is. Based on your pics I dont think ur fwhr is cooked. Its seems kinds okay and will increase by moving ur recessed maxilla forwards. I can analize ur fwhr for u (dont count on me, imma do it if i have the time and will but yeah probably imma do it) and ur bizygomatic to bigonial width. U need to upload a side peofile to better evaluate ur current state.

Can you calculate my fwhr and bizygomatic to bigonial width pls?

Just go to someone who actually does MARPE. But I can tell. EASY YES do MARPE. If you are actually smart you should figure out what are your problems. If you got class 3, palette width, protrusion, tipping yada yada. I dont got your mouth. Asking about MARPE but not showing your mouth is like going to a ortho without opening your mouth. Idiot stuff.

Why can't you get bimax ?

Why can't you get bimax ?
I dunno maybe because it’s a billion dollars and would destroy my current livelihood. I would like to look better, but I’m still happy with life outside of just looks. Bimax would compromise my life outside of lm that’s pretty great
