Would like to hear your guys thoughts on this plan.

My TMJ disc is displaced without reduction, and over the past year the joint issue has caused estimated 0.5 mm of condylar resorption along with significant pain, locking, bite changes, and facial changes. Because of that, I’m having TMJ surgery first to address the joint problem and hopefully stabilize everything before moving forward with orthodontic and orthognathic treatment.

I saw a surgeon who felt that getting the joints stable first made the most sense. After that, the plan is to do marpe because my intermolar width is only about 27 mm. The surgeon said a segmental Le Fort wouldn’t be a good option in my case because the amount of expansion needed would be too large to achieve surgically.

Once expansion is complete and the joints are stable, the final step is a double jaw surgery with clockwise rotation. I will also be having a septorhinoplasty at the same time as the jaw surgery.
