Maxillary downgrafting/addressing SFS

I have a mild case of SFS. Mainly this manifests in me having practically ZERO upper tooth show when talking unless I force it.

It seems that maxillary downgrafting is the way to solve this. Is this only done with double jaw surgery? I have minimal jaw recession but am willing to get DJS.

What are my options?
When my mouth is closed I can address the aesthetic issues decently enough with only a genioplasty. I want to address the lack of tooth show though.

If upper teeth don't show, there's no way around bimax... be careful though, downgrafting can be associated with increased scleral show, compensate that with advancement... and maybe add a vertical genioolasty, too

Downgraft or CW rotation. You can increase teeth show with lip lift, but obviously that won’t fix jaw deformities

My philtrum is not long. I already have a short midface. Is the downgrafting necessary? I read on jsf something like “for every 3mm you move the jaw you get 1mm tooth show”

Could be yes, but usually straight advancement is not a good idea. Do you have ct scan? If your occlusal plane is too flat you probably need CW rotation

are your lips bad from sfs? like bad lip seal?

If your occlusal plane points to the sky you need cw rotation, if it is too steep you need ccw rotation. But ct scan is needed

I read that just moving the jaws forward improves teeth show without downgrafting? And yeah i 10000% need a vertical genio. Thank you.

It does but it does not compensate for downgrafting, and CW puts you at danger of getting a chimp lip, we would need to see x rays
