I'm on a long term quest to find a surgeon who will do a DO (distraction osteogenesis) with RED (rigid external device) to improve my facial structure, like what is often done on young people with Crouzon Syndrome. But I'm just a standard normal person lacking forward growth. I want to move my face, excluding my nasal bridge, I would estimate around 5-6mm forward.

My plan is that first I'll do MSE, and possibly widen my mandible with DO (splitting it in the middle) at the same time. That will give me time to find a surgeon willing to do DO for midface advancement and possibly DO for growing the mandible forward. Most probably won't want to do it, because my purpose is for looking awesome and not primarily to fix any obvious functional problem.

I've always been aware that my zygos, orbitals, maxilla, and to a lesser extent mandible are deficient. Moving them forward would fix my thin appearing lips, especially upper, and possibly give my eyes more of a hunter appearance than a cuck appearance. I'm not a cuck though (I don't support anyone).

I'd be willing to spend 100,000 - 200,000 USD, and relocate for several months. Let me know if you know any good surgeons or have any advice or criticism, constructive or destructive or whatever. I'll document what I find in this thread when I can, over the course of months or a few years.

I guess most people here could benefit from a Modified Lefort 3 Distraction Osteogenesis with Rigid External Device procedure. It seems like most plastic surgeons just love to cut and graft bone and will lie to you and tell you that DO is not practical. It sure looks practical based on what I've seen. It morphs your skeletal structure naturally, and leaves you with virgin bones.

Ive only seen this work and been done with kids

Its better to just get lefort 2 and bsso with genio

It would cost way less too, probably £30-40k

Sadly, there are stability issues with DO with RED and consequently asymmetry that could occur with this

Fair, and noted. You're probably right. Although, I think a lefort 2 necessarily moves the nasal bridge forward, and my nasal bridge is already in an optimal location relative to my skull and I'll have to weigh the pros and cons if I find that someone is willing to do what I want.

I've seen DO advertised for maxillary advancement. But I don't think that would move the whole maxilla forward like an osteotomy.

In theory, it should probably work for adults too. It's frustrating how blue pilled most surgeons are.

That is useful info and I'll try to learn more. I'm hoping there are ways to mitigate that. I was under the impression that DO would be more stable than osteotomy.

Then just get modified lefort 3

That advances everythig apart from the nasal bridge and zygos i believe

I will consider that as well.

I'm pretty sure that modified lefort 3 moves the zygos as well, which is what I want. I read or heard somewhere that long term stability from DO was better than with osteotomy. Although forwardgrowth commented that there are stability issues with DO that can result in asymmetry, so I need to learn more about what's even possible.

Does anyone know how long I would need to wait to do a modified lefort 3 osteotomy / DO, after MSE espansion? I'll be looking for that info as well.

Just get a modified(high) lefort 1, and then a zygomatic sandwich osteomony.

This guy does it.

Seems overkill just get a bimax plus almond eye surgery with infraorbital malar implants. This will simulate the results without the risks.
