Can monobloc with mechanical distractors get botched?

Im only talking about midface advancement, not frontal bone.

If so what is the rate?

Ye bro, anything with monoblock and distractors can go sideways

Common issues:

Assymetry
Over/under advancement
Hardware problems
Infection
Soft tissue tension

rate is low ish if u go to specialized centers, like maybe 5-15% need revision depending on the surgeons skill and patient factors but never zero

mechanical failure or uneven distraction is not super rare

Usually could think more of repositioning the bones or even re-adjusting or replacing distractors

From what I know of this procedure

Sometimes you have to do a small osteotomy if where is first cut doesn't line up perfectly
revision doesn't mean starting from scratch but fixing whatever part went off.

Hardware swap, sometimes minor bone adjustments or soft tissue tweaks

Also sometimes they tweak soft tissue/scarring issues if needed especially if tension or asymmetry is causing problems
