My bimax pre-op report and scans (GTFIH)

Not posting any simulations to protect my identity.

Going with LACOMS in June, probably the best bimax surgeons ITW after Dr. Kasey Li. I don't want to skimp and I am willing to pay extra because I have crippling sleep apnea which totally fucked up life for me.

They are planning on doing a segmental 3-piece Lefort 1 which should give me about 5mm of expansion and 8mm of advancement, along with a CCW rotation of a few degrees (my dumbass forgot to ask how many) and a BSSO mandibular advancement of 12mm.

My plans after this surgery are to get a CT scan and have supra/infra-malar implants designed by giant and taken to Pagnoni, and then to have a conservative, closed rhinoplasty to make my nose less overpowering along with a septoplasty which should further improve my breathing.

i stated that i plan to get a rhino as soon as i am recovered from my bimax. im sitting around 18-20% BF and of course im working to get to 11-12. i just think i really should get bimax because im a fatigued zombie thanks to sleep apnea, plus my jaw is very downswung and i clearly am recessed

I'm going with Deepak Dugar in Beverly Hills, I like the conservative approach he takes and he is top tier, just very expensive

Seriously because for ideal aeshtetic results you want your frontal incisors around 2mm anterior of the vertical nasion line. You are right tho that 12mm is a significant movement already

Other than the desire for 1-2-mm of additional movement, there is nothing to add from my point of view. Keep in mind that we are just talkin about 1-2mm here and minimal differences like this needs to be considered on a case by case basis, i might be in the wrong here. Too much forward movement can look bad as well (monkey-ish). The 2mm stated by me come from Alfaros latest publication on Aesthetic Guidelines (0-4mm Incisors anterior of verticval nasion "Barelona Line" as he calls it), this paper did not distinguish between men and women tho iirc and i would recmmend staying conservative with male faces. you Are in The range fo 0-1mm now which is fine as well. The treatment plan looks solid imo. If you want to do further surgical looksmaxxing i would recommend considering after you bimax has healed properly and then going for custom implants.

because this clinic is the best in the world, i would rather pay extra (and i am able to anyways) to make sure i eradicate my sleep apnea than go to a shitty surgeon for free and deal with this issue later in life
