Honestly dont know why he gets shilled so hard here. He doesnt seem to personalise anything, he started planning during the consult. He may be the cheapest but you will get extremely conservative movements and they may not be necessarily the best movements for the issues you have. He tends to agree with whatever you ask for as long as it’s conservative - to me that is the worst of both world.

Alfaro:
Dont even bother with him IMO. He’s got a reputation for over advancement and in my initial consult it felt like he had only looked at my case for the first time during the consult. Important to note that he wont even start planning in your initial consult and if you’re looking for a web consult forget about it as the first thing he’ll say after he’s taken your money is “i have to see you in person to make any recommendations”. That would have been nice to know before i spent a weeks worth of rent on this consult. I did ask him how much advancement he thinks i would need and i forgot exactly what he said but i recall it being way more than anyone else was recommending.

Pagnoni:
Honestly a very intelligent guy. Really impressed me with his depth of knowledge and his preparation. He had multiple plans made for me before the consult. He is a good counterweight to some of the delusions on this site but at the same time he is not overly conservative with the movements he chooses to make. He has a lot of integrity and honesty as well - he will be firmly against surgeries that you dont need. He’s a bit on the expensive side but seems to be well worth it. I cant recommend him enough.

For now im getting some more opinions as one can never be too certain but im likely to proceed with Pagnoni for the reasons mentioned above.

Ramieri is hardly bluepilled from what I’ve seen. He’s happy to do 20mm+ advancement on the pogonion, and combine his bimaxes with jaw and infraorbital implants.

Idk what you look like so idk if it was good or bad if Ramieri was acting bluepilled towards u.

Im not affiliated with any surgeon. Ive been on this site for a long ass time and u can see from my post history that i used to be a mewing coper. Im just trying to be honest here about my own experience cause there are a lot of shills here for the two surgeons that i thought fell short of expectations. my issue with ramieri is mainly due to him just agreeing to perform any operation i inquired about without really considering whether it made sense or not. There wasnt really any pushback from him whereas pagnoni was a lot more opinionated. That can be polarising but i like to think that the average person would botch themselves given free reign and im no exception. I didnt get the impression that pagnoni doesnt care, more so that he isnt interested in mental gymnastics. That’s only my own experience, however.
 
Please avoid Pagnoni. I promoted him to people in the past before I knew any better, but he is a butcher and has ruined many lives. More info will come out over the next 1-2 years, I am sure.

He is impressive during the consultations as he will have looked at your scans and perhaps even made a plan. He talks a good game and everything sounds good in theory, but you’re playing an extremely dangerous game once you’re on the operating table.

Pagnoni has appeared to turn more conservative as of late, most likely due to the horrific botches, but don’t let this fool you into thinking he has integrity. He has none. Many people have had the same experience as you only to end up ruined.

Absolutely avoid Alfaro too. Ramieri is most likely quite overrated, but he is a better option than Alfaro and Pagnoni.

If you’re not happy with Ramieri, look for another surgeon to consult with.

I have not seen a single positive comment about Pagnoni on here for 4 months. What happened to this guys reputation?

All surgeons listed have multiple people complaining every year. 2 of them have a reputation management agency that gets things removed from reddit.

Do you have any advice to minimize the risk of someone ending up unsatisfied with their result - maybe some precautions someone can take before committing to a specific surgeon. Is it usually the patients fault? Or is it botches simply happen by pure chance on the operating table?

These things work in cycles. Ramieri is now flavour of the month. Until someone posts a bad result from him and then suddenly the hive mind will decide who the next goated surgeon is (maybe back to Pag or onto someone else).

Is bone grafting done during bimax usually? I it needed or not?
