Past Surgeries:

19 Feb, 2025
Rhinoplasty

My nose was a challenge, very thick skin, tip bulbousity, droopy tip and lack of any support.
I had some nose injuries in the past as well so my nose was crooked and I had trouble breathing.
They improved the breathing significantly.
Overall, I would rate it 6.75/10 mainly because it was a bit overpriced and the cosmetic result could have been better.
Definitely going for revision in the future.

Upcoming Surgeries:

May 20, 2026
Double Jaw Surgery + Genioplasty

What's confirmed:
At least 4* of counter clockwise rotation on both jaws.
5mm+ advancement on both maxilla and mandible
7mm+ genioplasty
Segmental Lefort 1 is not yet confirmed.

Round 2

Late 2026/Early 2027
Custom PEEK Wraparound Jaw Implant/Jaw Angle Implants + Premaxillary Implant + Infraorbital/Zygomatic Implants
My jaw surgery movements are limited because of my natural unfortunate teeth position and a combination of other factors.
I have what some asian surgeons describe as a “protruding mouth.” However that type of protrusion is often related to the shape of the maxilla and is typically addressed by removing a portion of the maxilla, often together with tooth extractions which is very fucking dumb.
After jaw surgery my face may actually look somewhat worse initially because the “protruding mouth” could become more noticeable.
That’s why my top priority is getting a custom premaxillary implant to balance my midface with the jaw.
Everything will be done with the same surgeon who performed my jaw surgery.

Future Surgeries:

Custom PEEK saddled supraorbital implant + canthoplasty, Rhinoplasty revision.

your protruding mouth could be fixed by ASO anterior subapical osteotomy or pre molar extractions

But I'm not asian and my structure is way different
So implants are the only solution

I don't think your mouth is protruded but it's an illusion caused by the very recessed chin.
I recommend decompensation to increase the overjet so they can advance the lower jaw more. Your teeth would be in a better position, better chin geometry and overall better profile.
Upper jaw advancement could make things worse.
Increasing the overjet again would be a horrible decision. my teeth are already positioned in an unhealthy way and ideally they should meet the lower teeth at a much smaller angle, increasing the overjet would worsen the protrusion of my teeth and mouth, which would also negatively affect my facial aesthetics.
Movements in my case are limited, to be honest the plan couldnt be any better than it is right now.
I definitely need some maxillary advancement here + my biggest airway narrowing is in the upper part so from a health perspective advancing the maxilla is necessary anyway.
I will need midface implants (premaxillary implants on the upper part) on top of the lefort after few months because even without touching my upper teeth my protrustion will look worse leaving my midface behind.
I had tons of consultations. in my case teeth "decompensation" or preparation before the surgery is useless.

1st pic - me, 2nd pic - healthy teeth position (tho small underbite is there)

"Increasing the overjet again would be a horrible decision."
oh yeah and i think you forgot the fact that im having counter clockwise on mandible and maxilla so now imagine those teeth being pushed even more forward and upwards

It's definitely not useless. Your lower front teeth are very proclined.

That it not what i meant. You did not understand what i said at all.

same thing goes for the upper teeth

4 degree counter clockwise will fix the lower teeth position a bit but how does that correlate with the upper teeth which could only be recessed here to match them and the fact that counter clockwise rotation will be done makes it even worse
it's just the unfortunate case of the way my maxilla is developed

Not really. Your upper teeth position is better. The scan you put in comparison to yours shows a very flat maxilla.
Increasing your overjet by moving the lower front teeth would be your best option.
