Need help w jaw surgery planning w pics

I have a jaw surgery case planned in five months. After pushing my ortho for a referral to a maxillofacial surgeon I went there a while back. He told me based on his estimations that he would give me an 11mm BSSO and that there was no need for a Lefort 1 or genioplasty. This really confused me as I thought much more advancement was needed. But since im not very informed I hope someone will be able to tell if this will suffice. I have attached some side profile pictures. First picture is relaxed mouth posture, second one has me putting my lower teeth up to my upper teeth (surgeon said this would emulate what it looks like after the surgery). I don't have any X-ray's yet, will post those once acquired. So my question is do u think 11mm BSSO will be enough and if not what would you suggest?

11 mm bsso, you must have a bad under bite

Remember, you cannot ever be over advanced with only a bsso, because your upper jaw will always limit your lower.

An 11mm bsso is a lot of movement, and it's your protruding chin that hides your lower jaw recession.

If we make your chin recessed as well, you can see the mandible recession more clearly.

If we make your chin recessed as well, you can see the mandible recession more clearly.

You are lucky that you have a nice chin, I don't.
but u dont think any advancement on the maxilla is needed?

Advancement of lower maxilla no need for LFII
My surgeon said performing any type of maxillary osteotomy wouldnt give me better results, why do u say I need advancement of the lower maxilla?

You'll need bimax for sure, because bsso will only make your jaw more downward if there is no cww rotation, and you can't just get bsso because you'll make your occlusal plane worse and you won't close your mouth properly, so i suggest you speaking to other maxillofacial surgeon.
 
I think all you need is a genio

Correct me if im wrong im new to this: The BSSO wil turn my jaw more downward, in turn increasing the amount of degrees of the occlusal plane, which is unideal. Adding a Lefort 1 with counterclockwise rotation will rotate the jaws up, decreasing the occlusal plane, which looks better. But do u think based on those pictures above my occlusal plane will be too high after the surgery? How would adding a Lefort 1 impact the results?If im honest i was also expecting some sort of Lefort 1 and i fear BSSO only will not satisfy. My surgeon is provided by insurance and i can't easily swap to another. There is a cephalometric x-ray in april, which will hopefully provide new information.

I mean, by the logic, if you only move the lower jaw with a cww, won't it affect your bite and make it worse?, you can try and talk with your surgeon about adding a lefort to it, i am no expert and the surgeon will always know better than some incels on this forum.

yeah ur right, he prolly knows best, but i will ask him about how the surgery will affect my bite next time i see him
