Canthoplasty, but I would say eyebrow tilt is far more important than canthal tilt.
However, your negative canthal tilt is borderline severe; therefore, it's a major failo. Your eyebrows seem to be slightly positively tilted, thick, dark, and low-set, so I would focus on shaping them to be more positively tilted and horizontally long.

i can give you tips: 1. get higher set cheekbones. this would result in a less noticeable NCT because the eye area will be more compacted horizontally.
2. try squintmaxxing, you need to try if it suits your face well (facial harmony is crucial). it could improve your cantal tilt a bit.
3. try to better your brows tilt. maybe youll think it doesnt suit you but its the best option if you have NCT (i have it too, sadly)
