orthodontist sheet (or something idrk)

I was scrolling through (either on here or TT or some shit, idfk) and I saw progress photos and the guy describing his process. I saw something I haven't really seen before but he had this word document full of shit wrong with his skull (shitty palate, underdeveloped maxilla, shit like that) from his orthodontist with the headline "Orthodontic Problems" or something, again, idrk. Does anyone know how I'd get that from my orthodontist? I kind of found it too good to be true but I heard they can just genuinely analyze everything about your skull and detail what's in need of correction or advancement. Obviously they aren't recommending surgery they literally are just giving you, on a sheet of paper, your orthodontic issues. If anyone could help w/ that (as in wtf i ask for or what its even called) that'd be great
