Palate Expanders for Aesthetic Benefits Even If My Orthos Disagree | MARPE vs. 2/3-Piece Maxillary Osteotomy

Hello,

I’ve been on orthodontic treatment for about 2 years. Both my orthodontist and another one told me that my upper palate is fine and I don’t need MARPE (in Turkey, we don’t have MSE or similar alternatives). But I still want it because:

Positives:
  • Right now, I have a 6-teeth smile. After braces, it will be an 8-teeth smile, but I want a 10-teeth smile, so I need a palate expander.
  • My lips are too narrow, and in Turkey, we don’t have lip expansion surgery, so a palate expander is my only option.
  • Palate expanders can might be give me more upturned eyes.
Negatives:
  • The orthodontists say I don’t need a palate expander my upper palate isn’t narrow (but not wide either).
  • It may make the nose bigger (but I’m already planning to get trimax surgery, so I don’t care about this one).
  • Orthodontists also say it might not even work.
Here’s my second question:
Which would be better for my issues MARPE or a 2/3-piece maxillary osteotomy?

The main problem is that my orthodontist doesn’t want to touch my upper jaw. I might need to find another ortho, but one of the best in my city also said I don’t need a palate expander. Still, I want it for aesthetic reasons.

In Turkey, FME will probably never be available, but I’ve seen some results with MARPE specifically in people who have NCT.

this isnt really true, it depends on the age of the patient, and their anatomy. fme and marpe do the same thing, its just fme is significantly more reliable in older adults. And results in nice expansion in younger patients. That doesnt mean its impossible for marpe to upturn someone eyes.

Guys, what about sMARPE? From my perspective, it should work because it’s surgically supported. In Turkey, we do have this option but I don’t like surgeries. Also, I’m planning to get Trimax, so maybe I could do it at the same time? I don’t know. I’m just sharing my situation and need some help.

I want a palate expander for the reasons I mentioned in my post.
I’ve paid all my debts to my orthodontist, but she says she doesn’t do palate expanders or any related surgeries. One of the best orthodontists in my city also told me I don’t need one.
In Turkey, we don’t have FME or MSE (or maybe I’m just unaware if anyone knows a doctor who can order them, please let us know).

I feel really bad and stuck. I want to get CCW + 2-piece + Genio Trimax, but my ortho says maybe I only need BSSO. I hate that.

consult with another orthodontist ofc. if u do end up getting it, maybe try a 2/3 piece lefort 1. its not worth the pain and time of mse if u dont even need it. better to expand surgically

Pain? I don’t care about pain. I only care about time and maybe a little bit about money. I can’t get the surgeries or procedures because of my ortho.
Guys, how can I find an orthodontist who does FME? Or maybe even MSE in Turkey?

Can GAINT help me to find a good ortho?

Are you talking about surgically asisted? If yes then it doesnt effect eyes or cheekbones coz the cut is made at lefort 1 area

that's what i thought, so marpe only choose for my benefits.

It’s good I’m happy for you. I’ve seen a lot of results, palate expanders are a game changer, especially for NCT.
 
I thibk it's possible to order some newer/custom type of expander abroad and get it installed in Turkey. The problem is asymmetry. I don't know designs which have the least amount of it, it's debatable. You need to look in related discord servers.

Ur right, but i need to find an ortho who wants to order palante expanders, cuz my ortho dont do that.
