I imagine complete castration (zero androgens including T) would obviously cause penile atrophy, but I doubt DHT blocking alone could. He should definitely get bloodwork though

Honestly some slight DHT inhibitors already exist in nature like in nettles or garlic for example, but I would never try the duasteride poisoning in the long run, same for isotretonoin on the liver function that could also affect ur testosterone production in an irreversible manner
