Planning for lower jaw surgery - need help

I’m currently mid-way through decompensation for orthognathic surgery and wanted to get some opinions on my OMFS’s current surgical plan.


Planned procedure:


  • Extraction of two lower premolars (no upper extractions)
  • BSSO advancement
  • Genioplasty

Ceph analysis shows:


  • SNA: 82.1°
  • SNB: 76.6° → retrusive mandible
  • ANB: 5.5° → Class II skeletal pattern
  • Wits: +8.5 mm → skeletal Class II tendency
  • MP-SN: 27.7° → skeletal deep bite
  • Interincisal angle: 111.9° → proclined incisors
  • Upper & lower lips: both protrusive relative to E-line
  • Mandibular plane: low (brachyfacial type, strong musculature)

You've a flat maxilla, go for lefort 1

should i extract upper premolars also to fix my flared teeth? marpe or expansion is not an option for me

what is your new venture about? it was surgerymax.cmf or smth?

So the difference is with company gigs you just design what doctors ask you to, right? No aesthetic fine tuning work by you unlike the personal service?

Sna is already ideal , will dogmax me
im also asian (half) and have 87.3 SNA. You should also do IPR for bimaxillary protrusion reduction or maybe more extractions. SNA is not indicative of dogmax in itself

also pretty sure you need your wisdoms removed, do that during your extractions
 
thats insane , alright ill ask for ipr for sure , but i am against extraction in upper jaw atleast? lower extractions and wisdoms will be removed i think

Yeah just say you want to address the bimaxillary protrusion and ask if IPR is enough. If they upper extractions are better I'de go for it though. But if yours isnt too bad, IPR should be fine.
 
What my omfs said today is, my philtrum is perfect right now, extraction in upper would cause midface recession which is not ideal? Ill just ask for ipr to make the flaring not that much

might be correct, looks like you'll pass lip assessments after jaw surgery. Ask your ortho as well
