Introduction

This thread has been a while in the making and is a result of my dive into the deepest levels of the looksmaxing community. I've hit the bottom of the rabbit hole and come to share my wealth of knowledge with you my friends, since I don't believe in gatekeeping. Please rejoice it may not be over for us (as long as you have the funds JFL)

Without further ado, let me introduce you to a drug known as teriparatide, it is classified as an anabolic and commonly used as a treatment for osteoporosis. It is a human parathyroid hormone (PTH 1-34) which is responsible primarily for calcium and phosphate regulation in the body. Teriparatide has the potential for extreme bone growth post puberty (note I am referring to facial bone growth/density... not enough for significant increases in height post puberty).​

Mechanism of Action

If we are to understand the potential uses for this drug fully, we must understand how it works in its clinical use for osteoporosis patients. Firstly, teriparatide promotes anabolism in the bone, this means it directly causes the formation of new bone matter within the body. In contrast to HGH (human growth hormone) and IGF-1 (insulin-like growth factor 1) it is bone-specific rather than full-body system-specific, a major difference we will discuss more later. It causes bone growth through modulating osteoblast activity (process where special cells known as osteoblasts build new bone tissue). There are 2 major steps in this process:​

Proliferation
Proliferation is the increase in preosteoblast cells. These are defined as cells destined to become osteoblast cells and are derived from mesenchymal stem cells (MSCs). MSCs are stemcels with the ablity to become multiple different types of cells, teriparatide acts on the Wnt/β-catenin pathway and causes more of these cells to become preosteoblast cells which in turn mature into osteoblast cells which build bone tissue.

Differentiation

Differentiation is the process in which these preosteoblast cells turn into osteoblast cells. Teriparatide increases the rate at which preosteoblast cells mature by upregulating runt-related transcription factor 2 (runx2) and Osterix (SP7). This also has minor side advantages through increasing alkaline phosphatase and collagen type 1 which give further bone growth.

Cell death prevention

The next big point within the mechanism of action is the anti-apoptotic effects, osteoblast cells die very very fast but teriparatide prevents this premature death via PI3K/Akt signaling. This inhibits caspase-3 (enzyme which causes the cell death) and promotes bcl-2 signalling (protein which prevents cell death). The result of these processes is an exponentially longer cell life for osteoblasts and therefore gives them the ability to build even more bone tissue.

Advanced bone synthesis

The human bone matrix consists of organic matrix (30% of total bone matter) and inorganic matrix (70% of total bone matter). Next, we will look at how teriparatide affects the 2 parts of the bone. The organic matrix is 90% type 1 collagen, the primary function is to give the bones tensile strength. Type 1 collagen production is increased via osteoblasts and osteoblast progenitor cells. As discussed earlier, these are made through a very complex process, which is supercharged by teriparatides. The inorganic matrix consists of hydroxyapatite, which is the most important mineral in bone matter. Teriparatide enhances the secretion of osteocalcin and bone sialoprotein, both of which are involved in the mineral binding leading to hydroxyapatite. Osteoids, combined with calcium and phosphate, harden and become bone structure, then they are turned into osteocytes by the osteoblast cell. As you can see, teriparatides are perfectly designed to boost the production of both parts of the bone more than any other compound on the planet.​


Other Effects


Along with the main effect of increasing bone mass, teriparatides also strengthen the bones, which reduces risk of fracture or breakage (especially advantageous for those with fragile bones). On the negative side, however, teriparatides can cause episodes of dizziness and high calcium levels in the blood. Since it is taken via injection it also has commonly associated sides suck as injection site pain and soreness.


Uses

Now that we understand the effects of teriparatide and how they work in the body, we will examine the possible uses for the purposes of looksmaxing. Firstly is the rapid regeneration of bone damage allows faster recovery from both minor and major surgery. I originally learned about this drug from a known figure in the cosmetic limb lengthening community who used it to recover from this surgery, as it causes significant bone damage. It can be used to recover with superhuman speed from cosmetic procedures. It can also be used to grow bone mass in your face and jaw. The human users whom I have spoken to note their jaw specifically being more prominent and better. The increase and advancement of bone in the skull will make a large impact on attractiveness for most people with low bone mass.

Human growth hormone and insulin-like growth factor 1 are the most commonly used compounds for bone growth. The main advantage of teriparatide over these two compounds is that teriparatide only targets bones, unlike hGH, which targets organs and muscle too, and IGF1, which targets cartilage. The other advantage is that teriparatide has a much greater degree of effect, while a lot of people take both hGH injections (or IGF1/hGH inducing peptides) alongside teriparatide, the overall effectiveness of teriparatide is much greater.


The Main Drawback

The biggest drawback to teriparatide usage is the amount of money it costs. Some users in the looksmaxing community, who I have consulted, who do not wish to be named, claim they spend thousands every single month, however, the results speak for themselves and make it well worth it. If you can afford it or find ways to get high-quality teriparatide for relatively cheap, it can be powerful.
