INTRODUCTION

This thread was made to understand the physiological and psychological effects of propranolol on different types of anxiety disorders, such as social anxiety and physical anxiety, by looking at some randomized clinical trials done in the past. While this is the main focus of the thread, you can also learn more about propranolol, what it does to different parts of the body, and when or under what conditions to use it.



WHAT IS PROPRANOLOL?

propranolol is a non-selective competitive beta-adrenergic receptor antagonist, meaning it competes with catecholamines like adrenaline at beta-1 and beta-2 receptors, thereby blocking their effects on the body, such as shaking, sweating, etc., leading to a reduced sympathetic response. This leads to reduced heart rate, less shaking, and sweating. Recently, Propranolol has gained increasing interest in psychiatry due to its potential effects on anxiety-related conditions. MECHANISM OF ACTION

Propranolol is a non-selective beta-adrenoreceptor antagonist, meaning it competitively blocks beta-1 and beta-2
adrenergic stimulation in the heart, lungs, and blood vessels.

Beta-1 Receptors are present in the heart, specifically on cardiac myocytes. When these receptors get activated by adrenaline and other stress hormones, this makes the heart beat faster and stronger. When propranolol blocks these receptors, it reduces the heart rate and the force of the heart's contractions. This helps to lower blood pressure and decrease the workload on the heart, making it easier for the heart to pump blood + reduced flushing and reddish skin because there is no vasodilation.
Beta-2 receptors are found in skeletal muscles and lungs. When stimulated by adrenaline, they can increase muscle activity and tension, which may contribute to tremors and shaking. They also cause vasodilation in the lungs. When these receptors are blocked, this leads to a small amount of vasoconstriction in the lungs. which is not a good thing for asthmatic people. At the skeletal muscle level, it helps reduce shaking, or tremors.


EFFECTS OF PROPRANOLOL ON FEAR MEMORY:
Before explaining the effects of Propranolol on the fear memory we have to know what the hell is it. Fear memory is when your brain remembers something that scared or threatened you before, so you feel scared again when you see or think about it. For example, if you were bitten by a dog, seeing a dog again or hearing a bark might activate the fear memory. Your brain recognizes the situation or cue that is similar to the past event, and this cause the release of adrenalin, making you feel scared again.
Reconsolidation of fear memory is a process where a fear memory, becomes temporarily unstable when it is reactivated. During this period, the memory can be modified, weakened, or even erased before it is stored again in the brain. disrupting or prohibiting reconsolidation help weaken or erase that memory. reducing the fear response. COOL RIGHT! this can be done through exposure therapy and certain medications.

WHEN TO USE IT?
Propranolol can be used in almost all cases if the desired result is to slow contractility and decrease the heart rate. In clinical settings, Propranolol can be used for angina, tachyarrhythmias, prevention of acute ischemic attacks, and migraine prophylaxis. It can also be used off-label for different types of anxiety disorders, like performance anxiety.

This experiment that was done in 2008 studied people with PTSD and tested how propranolol affected their fear memories. The participants were split into two groups: one took propranolol, and the other took a placebo. After 1 week they remembered a traumatic event, the group taking propranolol showed smaller physical responses, like lower heart rate and less shaking compared to the placebo group. This happened because propranolol blocked the reconsolidation of the fear memory, making it weaker over time. So, the experiment shows that taking propranolol during fear memory reactivation can help weaken it over time by stopping it's reconsolidation.

This experiment, which was done in 1981, looked at people with social anxiety. The treatment period was 4 weeks. The first group took propranolol 160-320mg/day and the other group took a placebo. The result showed no significant difference in physiologic response between the drug and placebo. This might be frustrating for some people, but we can't totally rely on this experiment because the two groups underwent a 4-week social skills training, which probably affected the results and reduced propranolol's effectiveness, in addition to the alcoholism and panic attacks + this was only one experiment conducted in 1981. So don't give up and believe that propranolol doesn't do shit to social anxiety. It does, but the effects vary from person to person.

This 1985 study tested whether propranolol helps people with specific phobia, like fear of snakes or spiders. They had 14 females take part and used a special method called a crossover trial, where each person tried different treatments. Some took propranolol, some took atenolol, and others took a placebo. They exposed participants to pictures of the feared object after giving the medicine. The results showed that propranolol did not change overall fear levels or anxiety. However, for people with high cardiovascular responses, propranolol was linked to lower anxiety ratings. The study also found that propranolol affected physical responses like heart rate and finger temperature, but it didn’t clearly reduce fear or anxiety overall. This suggests that propranolol might help with physical symptoms of fear, but not necessarily with reducing fear itself.

The study by Liu et al. (1991) investigated the effects of propranolol on individuals with dental phobia. 23 participants were assigned to receive either a single dose of propranolol (80-120 mg) or a placebo one hour prior to their treatment. The results showed that those who took propranolol reported significantly lower anxiety levels during the injection phase and experienced reduced overall pain and aversiveness compared to the placebo group. Interestingly, there was no significant difference in fear behavior observed by videotape raters between the two groups. The study concluded that propranolol effectively decreased anxiety and pain associated with dental treatment, suggesting its potential utility in managing dental phobia, without causing any dropouts or adverse effects.

CONCLUSION:

Propranolol is a non-selective beta-blocker, meaning it inhibits the beta-adrenergic effects of catecholamines like adrenaline, leading to fewer physiological symptoms such as heart racing, shaking, etc. It could be very effective for situations that cause heart racing, sweating, and shaking like talking to a girl, stage fright, etc. It also prevents the reconsolidation of fear memory, which weakens it. This results in less fear response over time.
