what's the difference between SARPE and SARME? like whats overall better for malocclusion?

No difference, just nomenclature. Months ago I read a strongly opinionated piece stating the case for why SARPE is an inappropriate term and SARME should be adopted. I wasn't moved.

Some people may use these to differentiate to specify a different technique or modifcation of SARPE but that is going to be surgeon-specific.

As for what is better? I will say MARPE (MSE specifically) followed by SARPE followed by Segmented LeFort I.

- MSE obviously has the benefit of being non-surgical and providing high expansion if desired. You risk waste of time and money if it fails which it reasonably can fail in men 18+
- SARPE can still provide some higher expansion varying on the technique used, and traditionally uses a tooth-borne hyrax (although MSE SARPE hybrid is now available). This is less pleasant and more expensive but unlike MSE it will almost definitely work as intended the first try.
- Segmented LeFort I is unstable and an even worse experience.
