acne won't get worse. you will be sensitive to the sun though.
before retin-a just try a easy skincare routine with just gentle cleansing, moisturizing and exfoliating 1-2 times a week with a salicyl acid (BHA) exfoliater.

If it’s as bad as you say it is then yes, use it. I typically wouldn’t recommend it to very young teens because they don’t understand you need to put factor 50 on, stay out of the sun/wear a hat in the sun/sunglasses etc but your skin will be glowing.
