Should I up my Accutane dose without telling my doc?

I've been on 20mg per day for about 1 year now after my life-ruining breakout, and I made good progress on Accutane, but recently I've not been satisfied with my progress. I feel like I've looked the same for like 2 months even though people regularly tell me it's getting better. My dermatologist barely gaf about me, I think, and she never considered (or talked about) upping the dose. I've heard it's possible that only side effects increase and progress doesn't speed up at all. Should I do it anyway?
