I have a Class III bite, asymmetry, and a slightly steep mandibular and occlusal planes. I need Lefort I with CCW, maybe 5 mm of advancement.

At first I looked into Korea because there they fix the skeletal discrepancy via surgery first, and then plan orthodontics from there. I’m 31, I like the sound of that much better than braces for a year.

But talking to these clinics is not going too well, CCW is like a foreign concept to them and they seem to want to set back my mandible to meet my upper jaw rather than move both forwards which obviously I won’t have.

My jaw and chin looks big from the outside so it’s confusing to even doctors that the inside is technically recessed and needs expansion. I don’t have sleep apnea, my airway is fine, but my teeth are crowded. So I need the surgery for medical/dental reasons as well as cosmetic ones.

So, are there any clinics in Turkey or anywhere else that can do Lefort CCW without braces first?

I know I need braces, but I want surgery ASAP and then braces after, not have braces both before and after which is an American/Western health industry money grab scam.

I'm in the same boat, I'd rather not decomp but I have to. However in most cases decompensation is necessary, especially if your surgeon wants to make decent movements and your teeth are causing issues

Not decompensating when you need to ends up with a shitty result. Take my words with a grain of salt since I'm not too well versed in this subject but that's from what I personally know

Surely they wouldn’t be able to accurately perform the surgery if they can’t match ur bite with straight teeth together once they’ve performed the osteotomy

Would need to see pictures and CT scans

We usually do surgery first at giant but in severe cases about 6 months of ortho may be needed first

Ramieri sometimes offers surgery first but it always depends on your occlusion, but if you are going for bimax then you can’t cut corners, you need to go all out and do anything it takes to get a good ascension

Would you call my case severe? With or without ortho first, is 5 mm or so forwards, 5 or so degrees of CCW, and some asymmetry correction doable for me?
