visited surgeon yesterday, he told me that my upper palate is 5mm narrower than the lower and first of all i need to fix my occlusion, so i’ll get a SARPE this or next month
why not MARPE? Well, i already have wide bizygo compared to my ipd and bigonial.
Then, most likely in summer 2028 i’m doing bimax, or mb trimax with ccw rotation.
I do it mostly for functional benefits, but i hope that it’s going to impact the aesthetics too, since my side is much worse than my front.

If somebody reading that thread does have any personal experience of those surgeries, then please tell me if i’m wrong🙏

maybe, i like my hump honestly, but from the front my nose looks pretty bad so that’s why
Ok man, aesthetically it's not ideal but you can decide for yourself I repped btw :)

dont need a bimax
except my fucking flat downgrown face i have tmj dysfunction due to asym of my mandibule and ramuses, i have narrow airways and i have masked with dental retrusion class 3 underbite, what may be the reason for NOT doing bimax

-you are not downgrown
-if anything it might only worsen a tmj disorder
-you need sleep studies to confirm narrow airways
-even if you have class 3 it's minimal
-the asymmetry is minimal, you are measuring it incorrectly cuz your whole skull is tilted
I doubt surgeons will agree to operate on you and even if someone does it will be extremely low roi

2) isn’t tmj the thing that should be fixed with djs?
3) doctor said that too, and yes i’ll do it. but i always notice that i breathe through my mouth, it’s just harder to breathe through nose
4) about the asym, i posted several threads with my face, and literally everyone said that the asymmetry nerfs me. my left ramus is noticeably longer that the right one and my occlusion plane is tilted, so idk how i measure it wrong

i honestly am in despair a little. i hope that it also might make my mfr better by rotating my jaws counterclockwise, because it’s long. and for sure i won’t make this surgery if i won’t be satisfied with the predicted facial changes, so my surgeon will scan my face and skull, and model the expected result

For sure i generally look for functional benefits, but i won’t do this surgery if it will make me look even worse than now

What do you expect to look like after your bimax? Your face is good now, and if you fuck up your face, there are damges beyond 10k and years in braces.

like several millimetres of moving my maxilla and mandible forward, to breathe and look better. and less noticeable asym, which should also fix my tmj.
and lastly, i started to notice the thing that my lower jaw wants to sit more forward naturally, and the retrusion of my lower teeth won’t be enough to mask my underbite soon. I don’t know, i can’t even hold the tongue posture because there’s not enough place
I actually feel like fucking defected person because all of that shit with my health

i feel like i already softmaxxed everything possible, except like my skin quality. and there’s no way to go much further without changing the structure of my bones and soft tissues. My base was way worse, now i get mtn ratings usually, but i still want to ascend
Like the shit stopping me are several ratios of my front, and literally everything about my side. Out of all surgeries i thought about, bimax or trimax are looking like the most worthy ones, and also they will be paid by my parents if i’ll decide

I need to learn about how doctors fix asymmetries in the jaws. I don't understand how they can do that with a cant and bimax.

Your face is kind of long. I remember that your midface ratio was no good.

I'm starting to think now that moving the jaws forward on a long face is not such a good idea, because it makes a long face longer. How much maxillary impaction can doctors do for you?

yeah it does, my tongue even starts to go between my teeth when i forget about that. I will most likely get SARPE, but i’ll also consider MARPE, if the simulated expected outcome will look good.

my mfr is around 0,92 to 0,93 and that’s not fixable with lip lift bc my nose is quite long and ctp is alright i guess.
but i noticed, that since my jaws are retruded, the place behind my upper lip literally has no support, it’s not that bad now bc i have braces on now, but without them, my upper lip just hangs down.
I hope that some CCW rotation, might create that support for my upper lip to flip a liitle or something so my mfr could be at least 0,94 or 0,95

How much maxillary impaction can doctors do for you?
first of all i should make my upper palate wider to get a correct occlusion, only after that they can tell me precise measurements and model expected result

Bimax is done only after 18, and i turned 17 3 days ago, so the next summer is also unavailable for me, and i don’t want to do
illegal surgery.

I’m not completely sure, but that’s exactly how it feels, i will ask my surgeon about that

I did a small research about that, FME also does widen bones connected to maxilla like marpe, but the main problem is that it’s very rare in my country and in my region especially, but i’ll go to ortho who works more with surgeon and ask that too

I may be wrong and miss something, but it looks like it works

I got like the same issue as you except in the other way slightly. projection pill is brutal cause even if you fix the side your front is gonna look nearly identical in most cases and it just matters way more. also from what I know the nasal base wont be more projected from bimax most of the time. Also marpe is cope, basically has no massive changes cause the rest of the face is fused.

I’d say my front doesn’t really look bad, so i mostly expect side and functional changes, and fixed asymmetry yeah
I also suppose that there might be slight changes of mfr, or nasolabial folds after bimax
 
you have good airways. 0 roi for bimax
i also thought that they were not narrow, but doctor told me to get a frontal scan, to confirm that airways are narrow and yeah they were. Nevertheless, i’m getting a sleep study soon.
And I want to get higher than mtn one day
