I have a narrow upper jaw so I’m going to see a doctor on the 10th of April to see if I can get a sarpe.
Would a sarpe and then lefort 1 (make my maxilla more prominent) paired with a bimax and genio(fix my lower third) make this morph slightly possible? Ofc I know that it’s impossible to even replicate this morph irl. Just want your thoughts on the direction of procedures that could make it look similar .

Parallel expansion is very rare

The main issue is the ramus length. Implants alone would not be able to fix this without causing dehiscence and looking extremely uncanny.

The best solution in your case is a custom osteotomy, akin to these images, followed by wraparound jaw implants after the osteotomy has healed.

This should be very easy for a maxillofacial surgeon, but I can imagine that many would be too bluepilled to do it.

people who say you need lefort are dumb, if you need to get marpe/sarpe definitely get it but a lefort is not worth it for you, the money and the suffering and you probably will. not even find a doctor who is willing to do one

Brother let me tell you in the UK they are extremely blue pilled . Most of these procedures I will have to do abroad no doubt in my mind.

I would not want to move an already half cut mandible forward within 6-12 months of doing that cut . But im not a surgeon either

Your jaws are not recessed. What’s happening in the morph is lengthening of ramus. You need implants for that. No surgeon is going to do bimax for you that is retarded

They may not be recessed but it’s certainly has some downward growth I was thinking if I get a bimax (ccw) and they help the angle it will improve my side. Im not any kind of expert in these things so let me know if what I’m saying even makes sense but that’s how I’m viewing it.

I respectfully disagree. Yes it is barely done and unknown. But no it is not risky and does not look weird. Whether it is worth it or not depends on the patient.

There is a small subset of capable maxillofacial surgeons who will use different cuts and osteotomies depending on the needs of each patient.

This is one example to fix asymmetry, by lengthening the jaw on one side

This is another example where this is done on both sides to create a masculine appearance of the jaw

As I said previously, there is very little that these types of osteotomies can achieve that bimax + implants cannot achieve. If you wish for a comprehensive advancement that addresses your entire lower face, then bimax is certainly the best option.

In the first example, this surgery was done because it was more cost-effective and carried less risk, and the only goal was to rectify the asymmetry. Some people just don't need a bimax.

I don't fully know the motivations behind the second example but it provides a better result than most jaw implants.

If you go to a maxillofacial surgeon who has experience with creative problem solving, and getting good results, you might be able to fix specific failos better, cheaper and safer than doing a bimax or implants. However, for people who require advancement and rotation of the maxilla, mandible and chin, a trimax is still the default option.

When it comes to ramuscels however, I still think this is the strongest option if you need an extreme amount of lengthening. Approximately 10mm of ramus lengthening is possible with implants, but if you need more, then you'll need an osteotomy that can provide more.

the facemetrics guy apparently got an inch of vertical height despite not having the worst ramus. Some people are just doomed to failure ig.


My main concern was the muscle dehiscence. Is it basically the same risk level as an implant? Or is it too case dependent and the surgeon should answer that.
