Does anyone know of surgeons that offer large advancements for a modest ($40,000 max) cost in either the U.S. or Europe?

Also, who are the cheapest surgeons regarding placing implants, and the best designers of them (Giant perhaps)?

Finally, has anyone here gotten a rhinoplasty for their nose tip? If so, who was your surgeon and could you provide before/after pictures?
