1.ugly as sin
2.average looking but mentally ill(autism,aspergers and etc.)
3Unfortunate circumstances(live in bumfuck nowhere, not enough girls their age wherever they live or no way to have access to them)

The amount of guys are incel simply due to looks is a rather small part, I would say maybe 1 out 10 guys is not attractive enough to even get some mediocre looking girl to be his girlfriend.
