2. The most important concept: Cumulative dose
One of the biggest misunderstandings about isotretinoin is dosing. Many people think the treatment depends on daily dose.
But in reality, derms focus on total cumulative dose over the entire treatment.

Most dermatology guidelines recommend a cumulative dose of:
120–150 mg per kilogram of body weight
Example calculation:
If someone weighs 70 kg

Then we go to moisturizer. U can buy any other good moisturizers, but i would recommend u to buy these products

Next, it's crucial to buy a good SPF, as u simply can't go outside without it. Most importantly, don't forget to reapply it every 2-3 hours if u do go outside. Buy 50+ SPF. In my case its La Roche-Posay

Also u should have any topical tretinoins. This will help ur skin actively get rid of pimples that will appear for the first 2 months. (U CAN USE IT ONLY AT NIGHT)

if u keep using the same aggressive products u used before, u will likely end up with:

Ur routine should stay very simple.

Morning routine:

3. SPF

Night routine:
1. Gentle cleanser

That’s usually enough. Also dont forget about lip balm, u need to reapply it every hour.

9. Will acne come back?

Statistics suggest:
• around two thirds of patients remain clear long term
• around one third may need another course

So, after u complete the course, u and ur derm wait a month for clear skin. Basically, u're doing absolutely nothing for ur skin. it's supposed to stay clear forever, and then u'll go into remission. Don't forget that acne is a chronic condition that has no cure, and if even a course of systemic retinoids doesn't help, it's all down to ur genetics. So yeah, that’s basically everything u must learned about isotretinoin and what to expect during the course.

is 20mg a day good dosage for very mild occasional breakouts and neck/scalp acne that are very mild also? i might up the dosage to 30 later for faster cumulative dose reach (8,5-9g)

So yeah, on a dose like 20 mg, the treatment is much longer, but usually with fewer side effects compared to higher doses

That’s usually enough. Also dont forget about lip balm, u need to reapply it every hour.
Overcomplicating ur routine while on isotretinoin often makes things worse


9. Will acne come back?

Statistics suggest:
• around two thirds of patients remain clear long term
• around one third may need another course

So, after u complete the course, u and ur derm wait a month for clear skin. Basically, u're doing absolutely nothing for ur skin. it's supposed to stay clear forever, and then u'll go into remission. Don't forget that acne is a chronic condition that has no cure, and if even a course of systemic retinoids doesn't help, it's all down to ur genetics. So yeah, that’s basically everything u must learned about isotretinoin and what to expect during the course.

BUMP BUMP THIS
Kind of general knowledge atp but I guess it's a good guide for someone that might be clueless regarding the topic. Also the skincare routine you dropped at the end to use during your isotretinoin course is unnecessary imo, one of the attractive things about accutane is precisely not having to do anything topical. And most importantly tretinoin while on isotretinoin is generally not recommended because tretinoin is kind of harsh and isotretinoin already makes the skin more sensitive while on it, also kinda unnecessary.
