Perhaps the surgeon tried to resolve that via CCW of his maxilla in the lefort 1. But it didn’t work. And now he also has a “reverse” smile

His lower third still doesn’t look good because his chin is still recessed. His chin needs to be moved forward (up to a line dropped from his lower lip) and downward (so that the skin under his jaw make a straight line).

even then, the skin under his jaw won't make a straight line due to:
-the knotch at his gonion
-probable low hyoid
-forward adam's apple
-likely other low soft tissue.

a genioplasty would not fix that.
