Tips for fixing acne?

Just turned 18 not sure what to do with my acne, I’ve tried a few things for quite awhile and none of it seems to work. I’ve also got a red carpet on my back that hurts to high hell whenever laying down or applying pressure. Wondering if there’s a way to reduce back acne.

at this stage go to dermatologist they will explain everything.no need to do it alone

It's retinoids, it's always going to be retinoids.

OTC: Retinol (pretty shit and weak), Retinal (pretty weak), Adapalene (the best OTC solution)
Prescription (can outsource it too): Tretinoin, Accutane, Tazarotene

Definitely try your luck first with Adapalene and see if you improve

Just run isotretinoin.
Especially if the acne isn't local to your face.

go to a derma dude, doing anything by yourself will be a money waste

It's retinoids, it's always going to be retinoids.

OTC: Retinol (pretty shit and weak), Retinal (pretty weak), Adapalene (the best OTC solution)
Prescription (can outsource it too): Tretinoin, Accutane, Tazarotene

Definitely try your luck first with Adapalene and see if you improve

start accutane ASAP. do not listen to other people here saying to try topicals first.
If you have acne on back too and your facial acne is already severe.
There is a chance it can get MUCH worse soon. You need to intervene now.

Start oral isotretinoin (name for accutane) 20mg a day immediatly and up to 40mg a day after a month.~

topical creams will do nothing for your bacne at this stage, you need oral treatments

Honestly, this, so many posts about the same thing that I don't even bother reading everything carefully anymore. Given that the acne is on the back too, and it's painful, oral solutions are the best bet 100%

What I don't agree with is hopping on a strong dose like 20mg immediately, I'd argue it's best to microdose it at first to avoid getting a nasty skin purge and irritation at the beginning, plus you never know how your body will react initially to high doses, Accutane can cause liver issues, mood swings n that kind of shit, better be safe than sorry imo.

20mg is not a strong dose. It is standard.
20-40mg a day is standard dose
40+ is high dose
less than 20mg a day or less than 60mg a week is a microdose.

microdoses are indicated when the acne is Grade I or II.
This is clearly Grade III acne with trunkal involvement.
It needs faster treatment and escalation to the highest tolerable daily dose
(usually 40-60mg for most people) so the total cumulative dose (in this case, i would reccomend 220mg/kg as that is the "ceiling" where benefit stops, but as its not nodulocystic grade IV acne you might be able to get away with 150mg/kg with the knowledge another course of accutane may be needed in the future).

The time it will take taking 40mg a day to reach the cumulative dose needed to cure the acne for OP if he is around 60kg for example is at least 8 months @ 40mg a day.

This needs to be started ASAP to prevent grade IV acne developing + permanent scarring (some of the scars from this severe acne can be untreatable) and secondary skin infections (bacne is a huge concern for this).




If the concern is with a purge happening you can pre-treat with 4 weeks of oral lymecycline antibiotic to reduce the bacterial load and use a probiotic supplement (there are guides for this on the forum and this is established dermatological practice in clinic).

after the 4 weeks you must wait 1 week before starting the accutane.



Important:
Do not use antibiotics to treat this without following up with accutane, the acne will come back worse
continue accutane until you reach a total dose of 220mg/kg (so if you are 60kg, if you have taken 40mg daily for around a year 13500mg net cumulative dose) OR your acne has completely gone away for month. You must continue accutane 1 month after acne clearance or until the full dose is reached.



If their are issues with side effects there are lots of different things you can do, one thing is pulse dosing where you take 80mg a day for 1 week of every month.


But to OP @Dom106 I highly reccomend starting accutane ASAP with the protocoll i said in my last message and continuing until your acne has been gone for 1 month

Isnt accutane purging?
 
Many doctor said using accutane are gonna have purge phase,dry skin,scaly skin
Every retinoid will have a purging phase, although the severeness of the purge depends on the strength of the retinoid. But why's that a problem? A purge is just that, a PHASE, won't last much longer than a month, yes there's going to be irritation, dry and peeling skin, but that's all worth when you have clear skin by the end of it.

Either way, if your acne is like the one shown in the pictures above, it's not worth getting Accutane for, topical solutions will work, with less side effects and for less money.
