Yep! 3mm downgraft, 9mm advancement at maxilla and BSSO following the maxilla. Some mandibular shaving on one side and like 1mm downward movement or so

Does he come across very experienced? Does he care about expansion at all cause I’m looking to get Sarpe with him but said if I’m a candidate and doesn’t look like he does it much

Yes, he is very experience and will go into great details if you ask him something specific, one of the best imo.

I'm going back there for dental rehabilitation in 4-6 months when everything has healed

Mirin, also got trimax coming up with Pera next week. Got any advice for planning with him before the surgery?

It all goes very fast, youre doing the surgery planning and the day after you go into surgery.

I asked a lot of questions, so perhaps have questions you want to ask written down, if you've had consultations before ask why he is doing X and not Y etc.

I dont think there's any secret in bimax recovery. Tons of people document it on youtube.

The swelling is overall the annoying part, and its normal to have some sort of identity crisis in the 4th week onwards period

He answered a lot of my concerns very well, and his movements made sense to me. He proposed 3-4mm maxilla, 8-10mm mandible, 5 degree ccw, and 6mm genio which seems like a fairly balanced plan for my moderate class II recession. Would you say he's got a a good eye for facial harmony though? And did he show you your expected post-op results via some sort of virtual surgical planning or simulation?

Yes I think so, he consistently posts good work. He *can* overadvance, he originally suggested 10mm advancement at maxilla to me as a minimum, and was like "this is the minimum, but it's a question whether you want 10 or 12mm", this would have been too much, Giant suggested 8mm to me and I went with 9mm which seems like a good balance. It's a big advancement though.

I'm getting more and more happy with my result as time goes, im 5 weeks post OP and my upper lip is still super stiff and very numb and I lisp when I talk, and no feeling in lower lip/chin.

Keep in mind, my case was very complex, I had already existing dental implants. Ramieri woudln't touch my case because he deemed it too risky, and I had a 3mm downgraft with hip bone grafting, and Ramieri doesn't even do that.

To illustrate overadvancement, this is for example Pera:

But honestly, I think this kind of advancement looks *good* on asian women, and much better than before, but each has their own taste.

And yes, you do VSP where you can see a simulation, but it is HIGHLY inaccurate and not worth relying on, a lot of surgeons has criticized that software and don't even show it to patients.

Damn, I really hope you get that feeling back but glad the result is getting better. Pera does do piezosurgery which is supposed to minimize soft tissue damage but I guess some level of permanent numbness is pretty much guaranteed from trimax. From what I've seen, his only unsatisfied patients seem to be female patients who got overadvanced, except that level of advancement typically looks pretty good on guys since we benefit from longer lower thirds and more anterior chin projection. Does he subscribe to any particular aesthetic school of thought? Like how sailer and alfaro are big advocates of anteface. I'm personally a fan of Rickett's line, golden ratio, and facial thirds that most orthognatic surgeons seem to follow and from what I've seen Pera seems to follow too.

Glad to hear that's the case. I was partly worried he'd compromise on function due to the surgery-first approach but this confirms the good feedback I've been hearing from his other patients. Dude might just be the next big name in orthognatic surgery, if he isn't already.

Appreciate the thorough responses. Feeling very comfortable about my decision to go with Pera now since this shit was definitely not cheap and surgery in Turkey is always a bit unnerving. Hope ya ascend, and I'll be following in your footsteps very soon.
