High malar fat pad mass contributes to nasolabial folds and an un-lean facial appearance.
Excessive malar fat pad thickness can be due to high body fat percentage, dogshit fat distribution genetics, and age-related saggage.
Example:
Malar fat pads that protrude ahead of the back of the nose look unaesthetic and fat/bloated.
Example:
Maxillary recession can exacerbate or create the appearance of excess malar fat pad protrusion, which deepens nasolabial folds and makes the surrounding area look bloated/fat.
Examples:
Male models almost universally have thin malar fat pads, forming a backwards-slanting zygo in 3/4 view. Combined with forward-slanting noses, the halos of both leanness and forward growth are optimized.
