They are left behind. You are only advancing lefort one area and mandible. You'd need implants to bring them forward too otherwise in most cases they won't show like pre op. The increased tissue stretching/ perceived leanness will come from jaw and chin.

But if ur cheekbones r really wide it wont affect the width right? Due to idk extractions and braces before bimax or something like that? The actual size of cheekbones doesnt change right?

Nope size and width will ofc be the same it only changes the projection.

Oh yah projection relative to the lower jaw. Makes sense now. But the actual measurement in all dimensions are same
