Sergio Oliva had all 4 of these things. Obviously he's enhanced, but his muscle insertions and frame are legitimately 1 in 7 billion.

Notice the super thick bones and extremely long and full muscle insertions. His biceps and triceps go straight in his elbows. His forearm insertion goes all the way to his wrists lol.
