What accutane cumulative dosage is optimal?

I've seen a lot of people and google say 120-150 mg/kg, but theres also a study showing that:

"In the lower-dose treatment group (<220 mg/kg), the relapse rate was 47.4% (95% CI, 32.3%-63.0%) compared with 26.9% (95% CI, 18.3%-37.8%) in the high-dose group (P = .03)... we found that patients receiving 220 mg/kg or more had a significantly decreased risk of relapse."

Could it be worth it to even push past 220mg/kg to reduce the chance of relapse as much as possible, or would this not be necessary if using topical tretinoin?

Most likely scenario: You use a lower dose and don't relapse.
Less likely scenario: You relapse and it's minor.
Least likely scenario: You relapse and it's major, all you have to do is take more accutane. You don't "lose" cumulative dosage by stopping and starting.

Obviously the answer is to use a lower dose. Lower dose = lower likelihood of permanent sides. You don't want to be the person with permanently cracked dry skin/lips for the rest of ur life.

My lips have alweays been super dry and cracked and the skin on my lips peels regardless my whole life, so im not really worried about that. 2-3 petroleom jelly and forget

I also in general just have super oily skin and hair so I feel like a higher dose might be better for me.

At the same time I'll be using topical tretinoin after im done with accutane so that could be enough to prevent acne indefinitely, but I still feel like going to at least 220 mg/kg could be worth it.

1. You have no idea just how bad dry lips can get
2. I laid out why the risk-reward ratio doesnt make sense here for higher dosing. You can always take more later if necessary.

There's no further discussion to be had after this. If you don't know what to do at this point it's an IQ issue.

I would recommend that you take your daily dose until you completely stop getting acne.

Some people still have acne after meeting their cumulative dose, in which case they should take more. Some can cut their treatment short by a few months.

There's no way to give an accurate number for your personal target dose. Look at how your body responds.
*Also make sure you are taking your accutane with a decently fatty meal.
