What is the most extreme looksmaxing surgery?

Nothing comes close to LF3, they literally peel your face back and break your facial bones in multiple areas risking permanent blindness and serious nerve damage.

Surgical techniques/technology have developed so well in the past 100 years or so, that even invasive surgeries like bimax, orbital decompression, facial implants, and leg lengthening are almost guaranteed to produce great results with few complications. We should all be thankful that we live in such an advanced era where you can have your flaws fixed to become better looking. This wasn’t a luxury people had 100+ years ago. If you were born with a flaw, you had to live with it your whole life. That being said, surgeries like LF3 are still out of the question and highly risky. Perhaps surgeries like this will become safer in the future with improved technology

Never before in the history majority of population was in need for major plastic surgeries, so that they could lead a normal life. If anything, this is the worst possible time to ever live in regards to that.

the most dangerous and extreme surgeries are the ones that can ascend legit truecels to average. I'd mention orbital box osteotomy, skull osteotomies and Lefort 3 osteotomy. also the more common double jaw surgery, limb lengthening and orbital decompression are very hardcore.
