Firstly you need a better picture without lens distortion to evaluate this. I've notice negative canthal tilts tend to be extremely exacerbated by close up shots.

take the picture at least 1 to 2 meters away from you, preferrably using a mirror.

either way yeah, probably canthoplasty/pexy, maybe ptosis repair too.
