Why does Lefort 3 B&As look so weird sometimes?

Title. Why do some patients who’ve gotten Lefort 3 look like they have fetal alcohol syndrome? It has something to do with the nose but I can’t put my finger on it. Does somebody smart know the answer?

Usually Le Fort 3's are done on pretty deformic patients, often syndromes like Apert and Crouzon. They'll look weird no matter what and is probably why you've asked the question.

Even with a purely cosmetic Le Fort 3 osteotomy (rare but I know Dr Sinn did them in the US pre-retirement, moving a whole third of your face unnaturally will often look fake. And most aren't, some may need to be done by distraction osteogenesis which removes precision.

Like I see cases like this where the nose is botched, it’s more common when a kid gets it done but I think it may be because the person had a disease and their whole face was smushed. I think that if you have a normal skull then you won’t get the fetal alcohol syndrome face cuz most LF3 make people look normal.

Sadly, implants can't mimic forward movement of the nasal area. This is why Le Fort II + implants seems to be the best approach for many.
