A few possibilities:
1) He's interesting to look at but not physically attractive. A girl saying "i love you" in the comments section of a photo is much different than a girl agreeing to go on a date. It's similar to how women will comment that a guy is hot on a youtube video if they feel sorry for him or they want to encourage his political ranting because they agree with it. There's no loss or risk for a woman in this situation as it won't lead to a date.
2) He's got niche appeal. IG casts a net around the entire world for people that find your look interesting enough to look at, so even a very small percentage winds up being a lot.
3) He's using Bots. Some of the comments seem a bit spammy so it's always a possibility.
