They diagnose it by symptoms since that's literally how all psychiatric disorders, are diagnosed. Yet not all psychiatric disorders have known structural differences.
It's also simi proven that stimulants permanently improve some of the anatomical differences in people with ADHD when taken during the right period of development. The only reason we don't have that widely circulated as fact, is that during the study, they came to that conclusion. At which point continuing the study, by denning the control group stimulants was deemed unethical. It was stopped and cannot be done again.
