Cheekbone implant alternatives

Title. Are there any? ZSO seems to be one, but according to people's experiences (and results) it's not that good. Seems like so far the cheekbones are the only part which non-extreme osteotomies cannot alter. So we're stuck with short term iffy solutions.

Fat grafting is difficult to predict ❌
Fillers migrate ❌
ZSO is not worth getting ❌
Vollufilline is BS ❌
Lyophilized cartilage is not stable apparently ❌
LF3-LF2/1 Area is not being done by any surgeon❌

Implants are good but personally I'm implant-phobic (+not many good providers)

Any other ideas?

ZSO is for increasing the width of the zygos but gives you no anterior projection.

None of the procedures you mentioned are strictly interchangeable. It depends on the exact nature of your midface deficiency which of them could help. But. if say 5mm of malar projection is what you need, there is no alternative to implants and that it is a risk you would have no choice but to take.

Use MSE/MARPE to widen your palette and get more cheekbone projection. You can facepull at the same time to get forward projection.

ZSO only works for lateral projection. For infraorbital rim (upper maxilla) you can’t create anterior projection. The only option for those who are implant phobic like you is calvarial bone grafting (basically taking the outer casing of the skull and using that to create projection).

This method does not have a lot of control though- it is shaped by the surgeon, so with won’t have the computer accuracy that implants have.
