Fat around nl folds. cause?

He is a bodybuilder, and i guess low bodyfat. Still his face is sagging, and you can see that he is 31 already. Shouldnt his face be more defined and without this excess fat/skin? What causes these nasolabial folds? How could he get treated?

You can use fillers, but it'll be a shit result. I think a lot of it also has to do with excessive malar fat pads that eventually can droop

Facelift to remove excess saggy tissue + filers/fat graft for volume would probably work
