What is GB-115?


GB-115, marketed as Ranquilon, is a synthetic peptide that works as an anxiolytic via a completely different mechanism than benzos or SSRIs (benzos mog harder tho). Instead of hitting the GABA system, it acts as an antagonist at cholecystokinin (CCK) receptors specifically CCK-2 (CCK-B) and CCK-1. CCK activation is involved in triggering anxiety and panic states, so blocking it essentially puts a lid on that pathway.


This makes it interesting as a non-sedating, non-GABAergic option for anxiety management.



Regulatory Status & Dosing


Not FDA approved. Currently in Phase 3 trials for anxiety and adjustment disorders.
Approved and used in Russia not approved elsewhere as of now.
Standard clinical dose: 6 mg/day (2 mg taken three times daily), oral or sublingual.
Onset of noticeable effects reported around day 7, with effects potentially lasting 21+ days.


What Does the Research Actually Show?


Worth being upfront here: the research base is thin and comes almost entirely from one Russian group, so take it with appropriate skepticism, and im yet to try it and note personal experience on it.


Preclinical (animal) work:


Reduced CCK-4 and yohimbine-induced anxiety in rodents.
Efficacy varied by mouse strain (worth noting for extrapolating to human variability).
No tolerance development or withdrawal syndrome observed after long-term dosing in animals.
Preclinical safety data has been published.

Human clinical data:


One pilot study, n=25, generalized anxiety disorder patients, 6 mg/day for 21 days.
Showed substantial reductions in Hamilton Anxiety Scale scores and improvements in fatigue.
Big caveat: Single-arm, no placebo, no control group. You cannot conclude causality from this design alone tho. The improvements are promising but could reflect placebo response, regression to the mean, or other confounders.


Community Experience:

As-needed dosing: 2-4 mg situationally seems to be the common approach for acute use.
Daily protocol: 6 mg/day mirrors the clinical trial dosing.
Sedation profile: Notably low compared to benzos most people report feeling functional.
Reported side effects: Occasional headache, mild GI discomfort. No dependency or withdrawal reported in community use so far.
What's unknown: Long-term safety data is limited. We're largely in uncharted territory beyond the trial windows


GB-115 is a pretty novel mechanism for anxiety that isn't just another GABA modulator. The preclinical profile is encouraging especially the no tolerance and no withdrawal findings. The human evidence is promising but weak. Community reports trend positive with a clean side-effect profile.
