gyno consult for my surgery: should i remove my mammary gland completely or partially?

I just had a consult with a doctor.
His plan for my gynocomestia is:
-Fat removal
-partial removal of the mammary gland
but: if necessary, they'll remove it completely. He said there are risks that the nipple folds inwards with the complete removal of the mammary gland
-breast lift, if the skin will hang after fat removal
Risk: unaesthetic 360 degree scarring around both nipples

I told him I want a maximal aesthetic result and that they can decide during the surgery what's best for aesthetics

Completely, have them graft fat under the nipple to prevent collapse.
