Has anyone actually Seen Any Meaningful facial structural changes after 16-17?

I've certainly seen changes in terms of facial maturity, which has increased perceived attractiveness, in terms of bone growth, not much no.

Past this age, surgery, assessments, fillers, may be necessary depending on the subjective situation.

If we are being here realistically the only change in "facial structure" is due to shifts in fat pads and diffrent body fat + maybe an tan or improvements in coloring which can make bones more obvious outside that the actual bone changes are very minimal. If you have deformed bones at 16-17 you need surgery and also get all orthodontics you need in additon. This is then the only way you can realistically improve the actual bones. Your bones should be pretty good after that regardless of base though. So you should be fine.

I’ve had palate expander and braces at a very young age already. Intermolar width is 45mm

Definitely not. You're not fully developed into your facial structure. Past the age of 18, potentially worth it if it's a good assessment and isn't botched.
