how to thumbpull?

Are you supposed to turn your head down (chin touching adam’s apple) and then push with your two two tumbs on your hard palate (pushing maxilla out) while only pushing (thumbs are on the same spot) or are you supposed to massage with intetion and moving with thumbs?

I’ve researched many threads but I cant find answers
