We conclude that MK-4 present in food does not contribute to the vitamin K status as measured by serum vitamin K levels. MK-7, however significantly increases serum MK-7 levels and therefore may be of particular importance for extrahepatic tissues.

Considering that MK-4 derives from the endogenous conversion of phylloquinone, the specific tissue distribution of MK-4 is suggestive of local synthesis from phylloquinone. MK-4 is synthetized in testes, pancreas and blood vessels. In particular, the conversion of phylloquinone into MK-4 occurs either directly or by interconversion to menadione (K3), followed by prenylation to MK-4 [21]. Therefore, the biological activity of K3 is entirely dependent on its prenylation to MK-4.
