Infras vs Orbital Decompression and Lower Eyelid Retraction

Looking for feedback. I have PCT but slightly bulging eyes and scleral show. My maxfac suggests infras to raise lower eyelid and create a more deep set look. Oculoplastic surgeon wants to do the decompression + lower eyelid retraction. He says that implants often look unnatural or cause scarring. He says the orbital decompression often looks most natural.
Can pm pics if needed.

funny how no one responds to actual looksmaxxing posts. This is why I'm not worried about mainstream BP

I'd go for the decomp route
 
You should do what occulo plastic surgeon says since they are more experienced and specialized in eye cosmetics, Implants or fillers have good impact for people with sunken dark circle under eyes (give volume) but I’ve seen opposite/feminizing effect of making eyes look less deep set for some.

I've seen a very good and natural looking infra + supra result for bulging eyes. It also seemed to reduce scleral show. Plus implants address more than just the eye itself if you're actually lacking bone.

Who is your maxfac?

The thing is a different occuplastic said of is extreme and I only need lower eyelid retraction.
 
You could also do upper eyelid blepheroplasty along with lower lid canthoplasty to want to get more hooded look and remove scleral show without risking drastic changes with OD. But, from what I have seen, people that needed OD get dramatic improvement afterwards when they do OD. You have to make the call if your issue is mainly because of eyes are bulging out or scleral show due to lower lid laxity. Also, ask for surgeons experience in doing OD and knowing how much fluid they will remove as the impact depends on that as well.
 
better to augment the bone than shove the eyes deeper into recessed bone

I really don't know brah.

I got infraorbital saddled implants because there was no way I was going to undergo orbital decompression : shit is scary, brutal and only for people with grave diseases (often thyroid issues).

It did make a huge difference though, I don't have scleral show anymore and my eyes appear deepset now (maybe because I got supras too).


I don't know why the oculo pushes for OD though.It's more expensive so more shekels in his pockets maybe ?

I know Bernardini is not a fan of implants either but he doesn't recommend OD for everyone unlike Taban.

For me OD is a meme surgery. The best combo would be implants to create positive vector and then lower eyelid retraction on top that will be sustainable thanks to extra support created by the implants.

My eyes have pct and aren’t that bulging. But have scleral show. The scleral show is the main issue imo + lack of support. I’m leaning towards lower eyelid retraction + infras

Id say OD, cause the eyelid retraction can sometimes look uncanny, also OD will automatically reduce scleral show.
 
OD will cause IPD reduction in about a 1:2 ratio to the eye setback.
