What are the requirements for insurance to cover a bimax, and if it's covered by insurance i assume it has to be done is U.S. where they won't do it with the intent to improve aesthetics, but if i have sleep apnea, narrow palate causing restricted nasal airflow, and underbite is that a case where insurance would consider coverage for Lefort 1 + BSSO and is it even worth it to do with a U.S. surgeon.
