I know there's a fascination with higher-level LeFort osteotomies on here, so I thought I'd share the scant few results I could find. Would be interested to hear users' thoughts on them. @lurking truecel @Lefor3Laser @zemult @midfacedeficient @czechpsl

Most of these results are pretty underwhelming, still showing a mid facial deficiency.

Almost all had an LF2 osteotomy apart from the two Asian guys who had DO.

my responses all ended up embedded oh well. im no expert anyway. nothing really spectacular here though, and looks like everything that was obtained coulda been done with just an lf1 and implants

Don't worry about embedding.
The first example definitely wasn't just a LeFort 1:

Number 2 had a LeFort 2 fracture and had roughly a 6-7 mm advancement.

Again, 3 and 4 clearly had their naso-maxillary deficiency eradicated from the side profile, but the frontal view remains largely unchanged:

5 in fact had distraction osteogenesis; in LeFort 2, if you advance the nasal cavity - especially by 1cm+, the nose is likely to become much larger. You can tell he had LF2 because the central midface is no longer caved in from the side profile:

6 had an LF2 advancement of roughly 5mm. I don't think she had any palatial expansion though; the increased tooth show comes from the advancement alone here imo:

7 had distraction osteogenesis, literally a device, a bit like limb lengthening where a gap is incrementally created in the entire LF2 region for soft tissue and bone to progressively grow into. Most patients who have DO actually need orthognathic surgery afterwards to get the optimal occlusal plane and bite.

8 was the best result although I think he had other procedures in the interim. There's no way he wouldn't have suffered from nasal enlargement; needs septoplasty.

well yeah he looks like a baby bird had its soft skull smashed in its gunna take a million operations to get him looking semi normal 😵
then even once they get the bones right itd be years before the facial muscles balance out if they ever do 😵‍💫 not a lot of hope for him

Yep the devise used for DO looks like a medieval torture device. I remember him saying in a reddit post that he would habitually faint pre-op. I imagine the guy had horrific airway problems; goes without saying this is a purely functional surgery.

lefort 2 is such a hilariously over-talked about surgery. Only makes an INSANE diff if you have a deformity and by that point you will never be attractive cause base is so cooked

Surgeries don't confer linear benefits. It's far easier to fix actual problems than create haloes.

Because they don't understand that crude midfacial advancement surgeries can't replicate or trump natural growth.
