The mandible can be actually widened through distraction osteogenesis. Some "camouflage widening" can be done through surgery, as well, by inserting implants. Some minor real widening can be achieved during orthognathic surgery by means of a midline osteotomy.

Dental arch can be widening a bit by orthodontics. The amount of widening depends on the initial status of your teeth (including roots and bone surrounding them)

How much does the mandible widen per mm of forward movement in the IMDO treatment?

Hard to specify, as it depends on several factors. But maybe 0,5 mm of widening at the first molar per each mm of forward movement of the lower incisors.

This also means that the maxilla need to be widened more than in a BSSO or standard distraction (non IMDO mandibular distraction, or lets call it standard mandibular DO, where de cut is done like in the BSSO)
