What surgery would fix my lower third issue?

My chin area is very narrow so I thought about a custom jaw implants with a genio but I might be wrong here because its clearly the narrow part under it.. genio focused on width is not gonna cut it but wouldnt jaw implants exaggerate the effect?

You don't need implants. What u need to increase ur lower third height is a procedure called intraoral opening wedge bony genioplasty

Never heard of this could you explain me more about it? And I assume you mean width instead of height?

"An intraoral opening wedge bony genioplasty is a surgical technique used to increase the vertical height of the chin, specifically for patients with a vertically short lower third of the face. This procedure involves cutting the chin bone (osteotomy) and dropping the lower segment down to create a gap, which is then filled with a bone graft to allow for significant vertical lengthening."

How would that be the case for me? I dont have a short lower third

Except u do. Just look at your lower third/middle third ratios. They’re significantly low
Thats the angle in this picture but by making it longer it would give me even more oblong head shape as shown on this pic right here

JUst tell me this and il solve you, wouldnt a wraparound implant for the lower mandible be the better surgery,

If ur goal is just to increase mandibular projection, then absolutely. But it won’t increase ur lower third length
